Long Term Care of Virginia establishes and initiates a medical plan of care in conjunction with the facilities to provide the best care for our patients. We work constantly to revise and adjust this plan as the condition and status of our patients change. Our goal is to provide all of our patients with the highest quality care whether they plan to return home or require an extended or permanent stay.
If a Primary Care Physician is unable to follow his/her patient’s care in a long term care facility, we will assign a Physician and Nurse Practitioner team to help monitor the patient’s healthcare needs. Once the patient has been admitted into the facility, routine visits will be made as deemed medically necessary. Urgent visits will be made as quickly as possible. While we attempt to handle most medical conditions in the facility, occasionally it may be necessary to send a patient to the nearest hospital for a prompt evaluation.
A covering Physician and Nurse Practitioner are available 24 hours a day for emergency calls.
